How whiplash injuries are classified.

The QTF whiplash grading system is used worldwide. Grade sets the prognosis and the right intensity of care.

Grade 0

No neck complaint, no physical signs. Typically does not require treatment.

Grade 1

Neck complaint of pain, stiffness, or tenderness only. No physical exam signs. Excellent prognosis with conservative care.

Grade 2

Neck complaint plus musculoskeletal signs (decreased range of motion, point tenderness). Good prognosis with 6 to 12 weeks of chiropractic and exercise.

Grade 3

Neck complaint plus neurologic signs (decreased reflexes, weakness, sensory deficit). Requires medical evaluation first, often imaging, possibly co-management.

Grade 4 (fracture or dislocation) is a medical and surgical case, not a chiropractic one.

Evaluation on day of visit

Mechanism, range-of-motion, orthopedic and neurologic exam. Digital x-ray if the exam calls for it.

Treatment begins same visit

Soft-tissue work, low-force adjustment (Activator or drop-table in the first week), ice and guided movement.

Progress measured, not assumed

Range-of-motion and functional measures remeasured at weeks 2, 4, and 8. Treatment adjusted based on findings.

Documentation to adjuster

Initial report, ongoing notes, and final narrative prepared in-house and sent to your insurance carrier or attorney.

What is whiplash?

Whiplash is a neck injury caused by rapid back-and-forth acceleration of the head, most commonly in a rear-end motor vehicle collision. The injury affects muscles, ligaments, joint capsules, and nerves of the cervical spine. Symptoms can include neck pain, headache, upper-back pain, restricted rotation, and sometimes shoulder or arm symptoms.

Why do symptoms get worse on day two?

Adrenaline and shock often mask pain immediately after a crash. Inflammatory responses in injured soft tissue build over the first 24 to 72 hours, which is why whiplash commonly feels worse on day two or three than on the day of the collision. This is one reason we recommend evaluation within the first week even if you feel fine initially.

Does chiropractic help whiplash?

Yes, for most whiplash cases. Research supports conservative care including chiropractic adjustment, soft-tissue work, and exercise as first-line treatment for grade 1 and grade 2 whiplash injuries. Grade 3 injuries with significant neurologic findings or suspected fracture require medical evaluation first.

Will my auto insurance cover it?

Yes. In Pennsylvania, your auto insurance PIP coverage pays for medical care related to a crash regardless of fault. Our office bills your carrier directly. Most patients pay nothing out of pocket for whiplash care.

How long does whiplash take to resolve?

Most grade 1 and grade 2 whiplash cases resolve within 6 to 12 weeks of conservative care. Patients who delay treatment, have higher initial pain, or have prior neck issues may take longer. A small percentage develop chronic symptoms; these cases benefit from early, consistent care.
